Management of Transverse Diameters in Surgical Patients

Transverse problems are a common occurrence in patients with dentofacial deformities requiring surgical correction. Modern orthodontic treatment techniques offer various options for their management, including orthodontic expansion, MARPE, MISMARPE, SARPE, and surgical maxilla segmentation (tripartition). Each approach carries specific benefits and contraindications. In cases where the deficit is isolated, the choice is guided by precise treatment protocols based on factors such as patient age, as well as the magnitude and type of the transverse discrepancy. In surgical patients, however, management is dictated not only by the correction of the transverse discrepancy itself, but also by concomitant skeletal problems. Furthermore, it is influenced by the need to optimize orthodontic presurgical preparation for subsequent surgical movements while minimizing the number and invasiveness of procedures. Additionally, proper management of the transverse dimension plays a crucial role in long-term treatment stability and final aesthetic and functional outcomes. This presentation aims to outline diagnostic and management flowcharts for transverse discrepancies in surgical cases, integrating them into broader, more complex treatment plans to provide participants with useful, easily applicable tools for daily clinical practice.
